圖書館閱覽證網上申請表

You are very welcome to visit our library during normal opening hours.
The visitor may have access to the Library to consult our collections for reference use only.
An application form will need to be completed for the registration to be processed.
The applicant should apply in a duly completed application form to be sent to library at least 3 working days in advance of the proposed visit date.
A confirmation letter will be emailed to you after approval.
The applicant should present the proof of your identity for registration at the KMS Library Circulation Desk.
A Library Reader Card will be issued which should be returned to the Circulation Desk on leaving.
All Library visitors are required to comply with the Library rules and regulations.
